ABSTRACT

Objective:To ascertain the endemic status of paragonimiasis in Zhejiang Province.Methods:From 2005 to 2020, 2-3 villages in 1-2 counties (cities, districts, hereinafter refferred to as counties) in historical endemic areas of paragonimiasis in Zhejiang Province were selected for monitoring each year. In each village, 50 to 150 local residents were selected as monitoring subjects, venous blood samples were collected, and serum Paragonimus antibody was detected by enzyme-linked immunosorbent assay (ELISA). Fifty to 100 intermediate hosts crabs or crayfish were collected in each village, and the infection of Paragonimus metacercaria was detected by crushing precipitation. Results:The positive rate of Paragonimus antibody was 2.9% (94/3 297); 3 929 crabs or crayfish were divided into 2 749 groups, 790 of which were found to have Paragonimus metacercaria infection, with a Paragonimus metacercaria infection rate of 28.7%. Conclusions:Paragonimus transmission chain exists in some counties of Zhejiang Province, which still has the potential risk of Paragonimus epidemic. Therefore, it is necessary to strengthen monitoring and carry out extensive health education to improve residents' self-protection awareness.

ABSTRACT

Objective To analyze the mortality level, death causes and trend of residents in Heqing Community of Pudong New District,Shanghai,and to provide a basis for formulating disease prevention and control plans. Methods Data of death surveillance of registered population in Heqing Community, Shanghai from January 1, 2013, to December 31, 2019 were collected. Descriptive epidemiological method was used to analyze the population structure, mortality rate, death cause composition and cause order of death of the residents in Heqing Community from 2013 to 2019. Results From 2013 to 2019, the aging level of residents in Heqing Community of Pudong New District each year was much higher than the aging level (16.15%) of the Sixth National Census, and the aging level remained an increasing trend. The death rates of residents in Heqing Community, Shanghai from 2013 to 2019 were 772.62/100 000, 751.59/100 000, 814.37/100 000, 814.43/100 000, 803.64/100 000, 736.52/100 000, and 757.81/100 000, respectively. The average annual mortality rate was 778.51/100 000, and there was significant difference between the male and the female (P<0.05). From 2013 to 2019, the top four causes of death were circulatory system diseases, malignant tumors, respiratory diseases, injuries and poisonings. There were significant differences in malignant tumors and respiratory diseases between the male and the female. Conclusion The aging of residents in Heqing Community of Pudong New District was serious, and the main cause of death of the population was chronic non-communicable diseases, which suggests that local health departments need to strengthen prevention and treatment of chronic diseases, and at the same time,pay attention to other external hazards such as traffic accidents and accidental falls.

ABSTRACT

Objective:To observe the effect of hypoxia on the dedifferentiation process of chondrocytes in vitro and explore the role of collagen prolyl 4-hydroxylase (C-P4Hs).Methods:Chondrocytes were treated with COCl 2 for different concentrations, and selecting the optimal COCl 2 concentration for hypoxia inductionwas100 μmol/L, the mouse costal chondrocytes were divided into the normal oxygen group and the hypoxia group, and the indexes of the 3rd generation 0.5-72 h and the 1st, 3rd, 5th and 7th generation at 72 h were detected respectively. The proliferation rate was determined by CCK8 method and cell count, and the dynamic changes of HIF-1α, P4Hα1, P4Hα2 and Col II in each group were detected by RT-qPCR, IF and Western blot. Results:Costal chondrocytes were cultured under different concentration of COCl 2 for 48 h. When COCl 2>150 μmol/L, the proliferation rate ( P<0.05) was significantly decreased. Normal oxygen and hypoxia induced rib chondrocytes for 0-72 h, and RT-qPCR showed significant increases in P4Hα2 and Col II mRNA in hypoxia group ( P<0.05). IF showed that HIF-1α and P4Hα2 accumulated in the nucleus under hypoxia, and P4Hα2 gradually entered the cytoplasm from the nucleus. Westernblot analysis showed that HIF-1α and P4Hα2 protein expressions were significantly increased in hypoxia group ( P<0.05). The expression of Col II protein in hypoxia group ( P<0.05) increased at the induction stage. CCK8 and cell count results showed that the proliferation rate and cell number of each generation in the hypoxic group were significantly increased ( P<0.05), and there was still potential for proliferation when the cells were transferred to the 6-7 generation. RT-qPCR showed that hypoxia group each generation cells P4Hα2, Col II mRNΑ were significantly increased ( P<0.05). Westernblot results showed that HIF-1α, P4Hα2 and Col II protein expressions were increased in each generation of hypoxia group ( P<0.05). ConcIusion:Increased expression of P4Hα2 through hypoxia induced HIF-1α can accelerate post-translational modification of Col II in chondrocytes and increase synthesis and accumulation of Col II. P4Hα2 may be responsible for increased proliferation rate and delayed dedifferentiation of chondrocytes cultured in vitro under hypoxia condition.

ABSTRACT

<p><b>OBJECTIVE</b>On the basic therapy, to assess the clinical effects of dynamic scalp acupuncture, scalp acupuncture combined with proprioceptive neuromuscular facilitation (PNF) therapy and simple PNF therapy for upper limb motor impairment in ischemic post-stroke spastic hemiplegia.</p><p><b>METHODS</b>A total of 90 cases were randomly assigned into a PNF group, a dynamic scalp acupuncture group and a scalp acupuncture group, 30 cases in each group. Basic therapy and PNF therapy were applied in the three groups. PNF therapy was used during scalp acupuncture in the dynamic scalp acupuncture group. PNF therapy was applied after scalp acupuncture in the scalp acupuncture group. The points were the upper 1/5 and middle 2/5 of (MS 6) and (MS 7) at the lesion side, the hemiparalysis contralateral side. The treatment was given for 6 months, once a day and 1 month as a course. The modified Ashworth scale (MAS), the Fugl-Meyer motor assessment (FMA) and Barthel index (BI) were observed before treatment and 2 weeks, 1 month, 3 months, and 6 months after treatment.</p><p><b>RESULTS</b>The MAS 1 month, 3 months and 6 months after treatment were improved compared with those before treatment in the three groups ( all <0.05), and the MAS results in the dynamic scalp acupuncture group were better than those in the PNF and scalp acupuncture group (all <0.05). The FMA and BI scores 1 month, 3 months and 6 months after treatment were higher than those before treatment (all <0.05). The FMA Scores in the 3 time points and after treatment in the dynamic scalp acupuncture group were higher than those in the other two groups (all <0.05).</p><p><b>CONCLUSION</b>PNF therapy during scalp acupuncture can relieve the spasmodic condition of patients with upper limb motor impairment in ischemic post-stroke spasmodic hemiplegia, and improve the limb function and life activity, which is better than PNF therapy after scalp acupuncture and simple PNF therapy.</p>

ABSTRACT

Objective To explore the correlation between serum lipoprotein-associated phospholipase A2 (Lp-PLA2) level with the occurrence of ischemic stroke and prognosis in the patients with hypertension . Methods Eighty-eight patients with hypertension in this hospital from January 2013 to January 2016 were se-lected ,including 34 cases of simple hypertension and 54 cases of hypertension complicating ischemic stroke . The Lp-PLA2 level was detected .The severity of ischemic stroke was evaluated on 1 ,14 d after admission by adopting the NIHSS .The patients with hypertension complicating ischemic stroke were divided into the good prognosis group and poor prognosis group according to the NIHSS change situation .Moreover the independent predictive factors of prognosis were evaluated by adopting the Logistic multivariate regression .Results The Lp-PLA2 level in the patients with hypertension complicating ischemic stroke was significantly higher than that in the patients with simple hypertension .The Lp-PLA2 level (OR=1 .523 ,95% CI=1 .323-1 .657) and NIHSS score (OR=3 .275 ,95% CI=1 .402 -6 .208) at admission were the independent predictive factors of poor prognosis .Conclusion Serum Lp-PLA2 level is closely correlated with the occurrence and prognosis of ische-mic stroke in the patients with hypertension ,which can serve an evaluation index for the occurrence and prognosis of ischemic stroke in the patients with hypertension and provides an objective basis for the clinical therapy .

ABSTRACT

Objective To investigate the correlation between DWI value of cervical cancer in stage Ib1-II A2 and its pelvic lymph node metastasis. Methods 103 patients with cervical cancer of stage Ib1 - IIa2 confimed by surgery and pathology in our hospital from January 2014 to July2016 were retro-spectively studied,78 cases of cervical squamous cell carcinoma group(lymph node metastasis of 22 cases,non- lymph node metastasis 56 cases);25 cases of Cervical adenocarcinoma group(9 cases with lymph node metastasis,16 cases of no lymph node metastasis). All patients with preoperative were performed MR routine scan and DWI examination. The ADC value(meanADC,minADC and maxADC)between the two groups and between the groups with and without metastasis were statistically analyzed. Results The value of ADC staging of cervical carcinoma had no correlation with clinical FIGO stage(Pmean = 0.847、Pmin = 0.393、Pmax = 0.219). The meanADC values、minADC value、maxADC value[(0.84 ± 0.09)× 10-3mm2/s、(0.78 ± 0.09)× 10-3mm2/s、(0.86 ± 0.07)× 10-3 mm2/s]of Squamous cell carcinomas was lower than that of adenocarcinoma[(0.98 ± 0.04)× 10-3mm2/s、(0.93 ± 0.09)×10-3mm2/s、(0.97 ± 0.23)× 10-3mm2/s]and the difference was statistically significant(P <0.05). The meanADC values and minADC value of cervical cancer within each group in lymph node metastasis group was lower than that of non- lymph node metastasis group,the difference was statistically significant(P <0.05). The maxADC value had no significant difference(P > 0.05). Conclusion The ADC value of cervical cancer has some correlation with lymph node metastasis,which can be helpful to predict the risk of lymph node metastasis.

ABSTRACT

Objective To evaluate the clinical safety and efficacy of CalliSpheres,a domestic drugeluting microspheres,in treating primary hepatocellular carcinoma (HCC).Methods A total of 12 HCC patients were enrolled in this study.Interventional chemoembolization with CalliSpheres was carried out in all patients.The preoperative and postoperative clinical data,laboratory results and imaging findings were retrospectively analyzed.Results The success rate of initial interventional chemoembolization in 12 patients was 100%,the median follow-up time was 7.5 months (6-9 months).One week after the treatment,both AST and ALT levels were obviously increased when compared with preoperative ones,the differences were statistically significant (P<0.05),although their mean values did not exceed the normal upper limit of 40 U/L.No statistically significant differences in Child-Pugh grading,creatinine level,hemoglobin,white blood cell count and platelet count existed between preoperative data and postoperative ones (P>0.05).At 3 months and 6 months after treatment,the disease remission rates (CR+PR) were 75.00% and 66.67% respectively,the disease control rates (CR+PR+SD) were 91.67% and 83.33% respectively.In 10 HCC patients whose preoperative AFP was ≥ 200 μg/L,the postoperative AFP levels showed a significant decrease.Three months after the treatment,the complete remission rate,disease remission rate and disease control rate in the CalliSpheres combined with Lipiodol sequential transarterial chemoembolization group were better than those in the simple CalliSpheres embolization group,but the differences were not statistically significant (P>0.05).Postoperative complications were mainly abdominal pain and fever.During follow-up period,the complications included pleural effusion (n=2),liver abscess (n=l) and lung metastasis (n=l).Conclusion The use of domestic CalliSpheres,as a drug-eluting microspheres,for the treatment of primary HCC is safe and feasible with satisfactory short-term efficacy.Its long-term efficacy and the effect of combination use of CalliSpheres and Lipiodol need to be further clarified with multicenter and large sample researches.

ABSTRACT

Objective To probe into the distribution characteristics of traditional Chinese medicine syndromes of polycystic ovary syndrome and lay the foundation of the establishment of polycystic ovary syndrome's TCM diagnostic criteria. Methods Relevant modern literature was researched from CNKI and Wanfang and PubMed during 1949-2016, and a total of 257 literature were retrieved and included into the database. The title, author, name of syndromes and symptoms were listed in Excel and Access, and the original syndromes and symptoms were standardized and combined, and the frequency more than 2% items were analyzed. Results The high-frequency TCM syndromes of polycystic ovary syndrome were: phlegm-dampness, kidney asthenia, kidney yang deficiency, kidney asthenia and blood stasis, phlegm-blood stasis, liver depression and kidney asthenia, depression of the liver generated pathogenic fire, blood stasis, kidney yin deficiency,qi stagnation and blood stasis, spleen and kidney's yang deficiency, spleen asthenia and phlegm-dampness, liver depression, liver depression and qi stagnation. The symptoms were summarized. The current situation of the researches on traditional Chinese medicine syndromes of polycystic ovary syndrome was discussed. Conclusions The nature of the disease were mainly excessive and deficiency syndrome. The main disease locations were kidney, liver and spleen. The pathogenic factors mainly were phlegm-dampness and static blood. There were deficiencies in the researches on traditional Chinese medicine syndromes of polycystic ovary syndrome which required improvements and further studies.

ABSTRACT

<p><b>OBJECTIVE</b>To establish an animal model of gastric cancer by long-term infection of Helicobacter pylori (H.pylori) and to elucidate the pathogenesis by proteomics analysis.</p><p><b>METHODS</b>Fifty male Mongolian gerbils (4-5 week-old and weighted 60-100 g) were infected with H.pylori and the gastric tissues were obtained after the infection at 3, 6, 12 and 24 months. Histological changes were evaluated by H-E staining of the gastric tissue sections. Detection of H.pylori was performed by in-vitro culture of fresh gastric tissue samples, PCR amplification of H.pylori 16s rRNA and localization by silver staining. In addition, proteins extracted from gastric tissue samples were subjected to two-dimensional electrophoresis (2-DE) at various infection time points. Protein spots with increased quantity over the course of H.pylori infection were selected and analyzed by LC-MS/MS. Finally, differentially expressed proteins between human gastric cancer tissue samples and lymph nodes were analyzed by real-time RT-PCR.</p><p><b>RESULTS</b>Colonization of H.pylori was observed in gastric tissue of gerbils as early as 3 months after H.pylori infection, and persisted till 24 months. Pathological examination of infected animals showed various histological changes including acute gastritis, atrophic gastritis, intestinal metaplasia and gastric carcinoma. Seventy-eight differentially expressed proteins were identified by proteomics analysis, among which 36 proteins were up-regulated and 42 were down-regulated. Analyzed by LC-MS/MS, ten proteins were identified, including lactate dehydrogenase, ATP synthase, fatty acid-binding protein, COX5B, peroxiredoxin-4, peroxide reductase, transgelin, succinyl-CoA ligase, keratin and protein disulfide-isomerase A2, among which transgelin, ATP synthase and lactate dehydrogenase were highly expressed in human gastric carcinoma and lymph nodes.</p><p><b>CONCLUSIONS</b>H.pylori infection induces the expression of transgelin, ATP synthase and lactate dehydrogenase, implying possible roles in the pathogenesis of gastric diseases including cancer.</p>

ABSTRACT

Syndrome differentiation treatment is the traditional model of diagnosis and treatment of diseases in traditional Chinese medicine (TCM). To establish scientific diagnostic criteria of TCM syndrome is one of the key points in TCM study. In this paper, the basic models of the relevant diagnostic criteria of TCM syndrome and existed problems were reviewed. The authors pointed out the advantages of establishing diagnostic criteria of TCM syndrome based on TCM syndrome factors and combination of disease in Western medicine system and TCM syndrome, in which not only the characteristics of the disease in Western medicine were considered, but also the complexity and flexibility of syndrome identification and convenient application in clinical practice were resolved. The basic model and frame of the above diagnostic criteria and the procedures and methods used in developing the diagnostic criteria were also described and discussed.

Objective: To research the character of Chinese Medicine syndrome and syndrome factor on COPD by analyzing the neoteric literature of COPD,and to make the names of Chinese Medicine syndrome canonical.Methods: To search the neoteric literature of COPD,build up a literature data-base by epidata2.0 software and analyze the data.Results: The number of the names of Chinese Medicine syndrome changes from 322 to 178.In the acute stage,the highest frequency of Chinese Medicine syndrome is: blockage of the lung by phlegm-heat,heat phlegm,blockage of the lung by phlegm-heat and the stagnant blood.In the stable stage,the highest frequency of Chinese Medicine syndrome is: deficiency of vital energy of lung and kidney,deficiency of vital energy of lung,deficiency of vital energy and Yin, deficiency of lung and kidney,deficiency of vital energy of lung and spleen.The position of the illness is lung,kidney and spleen.The character of the illness is phlegm,deficiency of vital energy,heat,stagnant blood and deficiency.Conclusion: The names of Chinese Medicine syndrome are multiform and need to be canonical.The main character of COPD in the acute stage is affected by outside illness factors.The main character of COPD in the stable stage is weak inside.The syndromes are different when the illness in the different stages.

ABSTRACT

Objective To assess the clinical value of transcatheter arterial chemoembolization(TACE) for the treatment of metastatic liver leiomyosarcoma(MLL).Methods CT triphasic scanning and TACE were performed in 8 patients with MLL,which was proved by fine-needle aspiration biopsy and immunohistochemical study.A follow-up of 1-3 years was conducted.Based on the enhancement degree of the hepatic lesion and the DSA manifestations of the lesion,which were observed both before and after TACE,and also on the survival time,the therapeutic effectiveness was evaluated.Results Of the eight cases,single metastasis was seen in 3 and multiple metastasis in 5.Excellent result was obtained in 6 cases and good result in 2 cases.Based on the imaging findings,marked effect was seen in 3 cases,good effect in 4 cases and no improvement in one case.The surviva1 rate at 1,2 and 3 years was 87.8%,50.0% and 37.8% respectively.Conclusion TACE is a safe and effective treatment for MLL.
